Travelling in climatic regions very different from ours with extremely hot or cold temperatures is a challenge even for a healthy organism. And even more for a patient with diabetes mellitus. In this text, the particular problems which can arise during such journeys have been described for such patients but also for doctors wishing to provide qualified advice in travel medicine. These recommendations were developed on the basis of the available literature but also on the basis of many years of personal experience of journeys to all seven continents, in deserts, tropical rainforest, and in very cold regions in the Andes, the Himalayan and Alaska. The specific health problems which can confront a diabetic during journeys in a desert, a rainforest or polar regions are discussed. Day to day health problems of a diabetic are not the subject of this paper.
